Right knee pain. The provider documents a possible meniscus tear and orders an MRI. No definitive diagnosis is established at this visit.
“I would code the meniscus tear because the provider ordered an MRI. Is that right?”
Look at the word possible. Is the tear confirmed? What is documented with certainty at this outpatient visit?
“The tear is only suspected. The right knee pain is confirmed, so I should look up the symptom instead.”
Exactly. The outpatient uncertain-diagnosis guideline directs you to code what is known at this visit. An MRI order does not confirm the tear.
Now look up knee pain in the Index, then verify the right-side code and instructions in the Tabular List.
Recognize uncertainty, apply the rule for the setting, and support your code with the documentation.

Why this program is free
Too many students are taught how to pass an exam before they are taught how to code. This program exists to close that gap. Foundation first, then exam.
